how to create subform and add it to body page

hi all,

iam new adobe designer.

in body page, have added 1 text filed , 1 button. based on user entered data in text field, want display fileds. want create 2 or 3 set of components , display sub components based on user input.

1. how create sub form?
2. default sub form components invisible, after click on button, show or visible subform.
3. how add sub form main/body page?

please 1 me.
